Spa Membership Financial Policy

1. Membership Commitment
By enrolling in a spa membership, members agree to a minimum commitment of three (3) months. During this initial period, cancellations are not permitted. After the initial commitment period, memberships may be canceled by written notice 1 week before my next scheduled payment.

2. Monthly Billing & Payment Authorization
All members must provide a valid credit card upon sign-up. The designated credit card will be automatically charged monthly for the membership fee. By enrolling in the membership program, members consent to recurring monthly charges.

3. Treatment Rollover Policy
Members may carryover treatment benefits for 1 month if they have an active membership. Any treatments not used within this timeframe will expire and cannot be redeemed or refunded. Treatment rollover is subject to availability.

4. Annual Membership Option & Discounts
Members who commit to an annual membership will receive significant discounts on membership fees and additional spa services. Annual memberships must be paid in full at the time of enrollment and are non-refundable.

5. Cancellation Policy
After the initial three (3) month commitment, members may cancel their membership with a thirty (30) day notice. Membership benefits will remain active during the notice period. If cancellation occurs before the notice period ends, the final monthly payment will still be processed.

6. Refunds & Transfers
Membership fees are non-refundable. Membership benefits and services are non-transferable and may only be used by the enrolled member.

7. Treatment Consideration
Members must receive a consultation to determine they are a good candidate for the treatment before scheduling the appointment. Members considering injectables may need to purchase additional sites or syringes to achieve desired results.

8. Policy Acknowledgment
By signing up for a medspa membership, members acknowledge and agree to the terms outlined in this financial policy. Failure to adhere to these policies may result in termination of membership privileges.

9. Credit Card or Financing Payment Disclosure
Services that are rendered and paid with a credit card, debit card or with financing are not eligible for disputes/ chargebacks with your credit card company. As a patient, you hereby agree that this credit, debit card or financing challenge agreement is irrevocable.
Should any financial dispute/ chargeback arise, you hereby allow the practice to provide your credit card company with any documentation needed to facilitate a resolution. This includes, but is not limited to, your medical records.

I understand that I am in full control of my payment, and if at any time I decide to make any changes to my credit/debit card information, I must submit to Bitar Cosmetic Surgery Institute the new card information before the next due date. If at any time I decide to terminate my membership, I am required to give a written notice 1 week before my next scheduled payment. I also understand no refunds will be issued after membership due has been charged. Change of payment method will not affect other provisions and terms of my agreement.
